Wow.  Check it out.  While searching for FWC Blazers, I ran onto this wonderful photo here on WtW.....

The NADAGuide listing in post 1 shows Flat Bed models in 2015 and 2016.  According to this announcement from Stan and this article in Truck Camper Magazine, they started in 2013 and models were Fleet, Hawk, and Grandby.

 

That TCM article mentions a build for a Mitsubishi Fuso underway at the time. This thread points to a very interesting article on it at Truck Camper Magazine.  (link to article)

As far as model name, it's called a 'custom Four Wheel Camper' or 'the Fuso Four Wheel Camper' and has a model year of 2013.

And then there are the FWC models with the Hard Top option.....

This thread from January 2010 describes the prototype and includes a Truck Camper Magazine interview (and more photos of the one above).

 

In the article Tom says the HT option is available for the mainstream models except the Falcon.

 

And this thread from November 2013 notes the demise of the Hard Top option.
